Search for pickleball and tennis elbow and you'll find two kinds of content: medical articles written for tennis players in 2009, and paddle ads promising your arm will feel twenty years younger. Neither helps you make decisions.

Here's our position upfront: we're a gear site, not a clinic. Nothing below is medical advice. What we can do is break down the load factors — the variables that determine how much work your forearm does on every swing — and show you which of those variables you can actually change with gear and habits. With that out of the way, let's talk about why pickleball loads the elbow in the first place, and what you can do about the parts within your control.

Why pickleball loads the forearm

Lateral elbow irritation — what players call tennis elbow — is associated with repetitive stress on the forearm extensor muscles, the ones that fire when you grip and stabilize the wrist. Pickleball is a repetition machine. A typical rec session involves hundreds of paddle contacts: dinks, resets, counters, speedups. Each contact is small, but the volume adds up, and the volume is exactly what most players don't track.

Four load factors do most of the work here. They stack, which is the part people miss. A player with a death grip on a head-heavy paddle playing five days a week isn't dealing with one problem — they're dealing with three multiplying each other.

Load factor 1: Overuse and ramp rate

The most common pattern we hear from players isn't "I bought the wrong paddle." It's "I went from playing once a week to four times a week in a month." Tissue adapts to load over time, but it adapts slowly, and a sudden jump in volume outpaces it.

This is the cheapest variable to control and the one nobody wants to control, because pickleball is fun and your Tuesday group needs a fourth. Commonly recommended practice is to ramp volume gradually — adding sessions over weeks, not days — and to treat consecutive-day play as a real cost, not a flex.

Load factor 2: Grip tension

How hard you squeeze the handle matters as much as what's on the end of it. The forearm extensors are gripping muscles. Squeeze at maximum effort for two hours and you've done two hours of forearm work before counting a single swing.

Most players grip far harder than the shot requires, especially under pressure. A commonly used reference: on a 1-to-10 scale, dinks and resets need something like a 3 or 4, not an 8. If your knuckles are white at the kitchen line, you're paying a tax on every single contact. Load factor 3: Swing weight

Static weight — the number on the spec sheet — tells you what the paddle weighs sitting on a scale. Swing weight tells you how heavy it feels when you swing it, which is what your arm actually experiences. Two 8.0 oz paddles can swing very differently depending on where the mass sits: weight concentrated toward the head raises swing weight; weight in the handle lowers it.

Higher swing weight means more momentum through contact (nice for power) but also more effort to accelerate and — critically — more effort to decelerate and stabilize, hundreds of times per session. Paddles in the ~120+ swing weight class, especially head-heavy ones, meaningfully increase arm load compared to paddles in the low-110s and below. Load factor 4: Vibration

Every off-center hit sends vibration up the handle into your hand and forearm. One mishit is nothing. A few hundred per week, on top of the other three factors, is a real input.

Core thickness is the main gear lever here. Thicker cores (16mm+) damp vibration better than thin power cores, full stop.   • Audit your grip pressure mid-game. Set a literal trigger — every time you walk back to the baseline, check whether you're squeezing harder than the next shot needs.
  • Cap consecutive days when your arm is talking to you. Soreness that builds across a week is information. Players commonly schedule rest days the way they schedule play days.
  • Check your technique on backhands. Arm-only backhand flicks, where the wrist and forearm do all the work, load the elbow more than strokes driven from the shoulder and trunk. A lesson is cheaper than a layoff.
  • Don't play through sharp pain. Soreness and sharp pain are different signals. For pain that persists or worsens, stop self-diagnosing and consult a medical professional — ideally before it reshapes your season.

What an elbow-friendly paddle actually looks like

If you're shopping with your arm in mind, you're looking for a specific combination of traits — not a magic paddle, but a set of specs that may reduce strain and vibration relative to what you're playing now:

  • Moderate swing weight. Roughly the 105–112 class. Low enough to reduce the work of accelerating and stabilizing, high enough that the paddle still does something on contact.
  • Thick core. 16mm or more. Thicker cores damp vibration and slow the ball down, which also buys you time — a side benefit nobody mentions.
  • Balance closer to the handle. Head-light or even-balanced builds keep effective swing weight in check even when static weight is ordinary.

Gear is one lever among several, and probably not the biggest one. Ramp rate and grip tension are free to fix and likely matter more than any purchase. But if you're already managing those and your paddle is a head-heavy, thin-core power build in the 120+ swing weight class, moving to a moderate-swing-weight, thick-core, dampened paddle is a rational change that may reduce the strain and vibration your arm absorbs every session.

What gear cannot do is treat an injury that already exists. If your elbow hurts today and has for weeks, the next purchase that makes sense isn't a paddle — it's an appointment with someone qualified to look at it.
